How do I submit a claim? What do I need?

If a patient has Fast Claims enabled, you need to send us the invoices, recent history (since Fast Claims were set up) and clinical notes about the condition & treatment. If you’re a referral vet, or the client has moved to you since Fast Claims was set up, please include a full history.

We also need you to confirm a few key facts about the claim:

  • Condition Details/Name

  • Symptom Start Date

  • Treatment Start Date

  • Treatment End Date

  • Invoiced Amount

  • Amount Customer Has Paid
